The Give Kids a Smile Mobile Dental Clinic will be at UTHS on Monday, December 02. They will be provide oral health education, dental exam, teeth cleaning, and fluoride and sealants if needed. Participants will also receive a toothbrush and floss. Completed forms must be turned in no later than Monday, November 25 at 3:30pm. Click on your preferred language to view and print the form: English or Spanish. Forms can also be retrieved from the school nurse's office.
